Social Networking Sites Questionnaire

Hi everyone. I am a final year psychology student and for my dissertation I am investigating effects of facebook use on health. I am struggling to find any valid and standarised questionnaires on use of social networking sites. If anyone knows of any such scale please help! Thank you.
